Medical Services


Skilled Nursing Care

Our Skilled nursing services are provided by specially trained registered nurses and licensed practical nurses that begin all cases with an assessment to obtain information to establish a plan of care. The services are prescribed by a doctor on an intermittent basis. The nurses make visits to your home to begin your recovery process. They monitor your condition, report your progress and any unexpected changes in your condition to your physician. Their services include teaching and administration of medications, wound care, intravenous therapy, catheter care, diabetic care, diet, personal care and teaching disease process and symptom management.

Rehabilitative Services

Rehabilitation services include physical therapy, occupational therapy, and speech therapy. Licensed therapists provide intermittent care basis in the home as needed for patients who have functional limitations or sensory deficits.

Physical Therapy

Physical Therapists are educated in understanding the interaction of all your body parts. Services provided by our Physical Therapists include: muscle strengthening, gait balance, improve endurance, educate on the use of assistive devices and prevent recurring injury. Over all aim is to improve quality of life for our patients at home.

Occupational Therapy

Our skilled Occupational Therapist will evaluate your self-care aptitude and leisure skills and implement social and interpersonal activities to develop, restore, and /or maintain your ability to accomplish activities of daily living such as eating, dressing, bathing, and other necessary occupational task.

Speech Therapy

The role of our Speech Therapist is to evaluate our patient swallowing abilities, assess patient speech, language and communication problems. Our Therapists specialist will work with you who have issues with eating and swallowing, cognitive attentions, speech controls, vocabulary enhancement and articulation exercises.

Social Services

Our social services professionals assess the social needs of our patients. They provide ongoing assistances and community resources that are appropriate of our patients’ hardship. Services include evaluation of social functioning, particularly in relation to medical issues, initiation and follow-up of referrals to other community resources as appropriate.

Home Health Aide Services

Home health aides work under the supervision of a registered nurse or therapist. Services primarily consist of assistance with personal care and are unique for each patient, according to your needs. These services may include grooming, bathing, oral care, foot care, shampoo, shave, skin care, dressing, light housekeeping and meal preparation.

Companion Service

Living alone can be wonderful when you’re able to get out and about. But when illness, age, or a surgical procedure leaves you housebound, the solitude may be less welcome.

Through our non- medical service, we offer companionship services. Our trained home health aides are skilled and credentialed and can deliver a wide range of services, but we also offer much-needed companionship, providing you or your loved one with someone who is helpful, personable, and friendly. In addition, our home health aides are trained to look for changes in emotional, physical, or mental conditions.

Bathing / Grooming

Illness or injury can turn the simplest tasks, like bathing, combing your hair, or attending to other personal needs, into difficult chores. Perhaps you or your loved one has suffered a broken bone or had a stroke, or maybe an elderly family member has trouble with balance, so getting in or out of the bathtub or shower is a problem.

Through our non-medical services, we offer bathing and grooming services for people who are unable to perform tasks of everyday hygiene and personal appearance. Our home health aides can help with bathing, shaving, oral hygiene, and dressing.

Laundry Service

Perhaps an elderly relative is afraid or going up and down narrow basement stairs, carrying a load of dirty linens or a basket of just-washed clothing. Or maybe a medical condition or simply being older means you or your family member can no longer fold laundry.

If you or a loved one is unable to take care of doing personal and household laundry, our affiliate company, Partners in Care, can help. Our dedicated, credentialed professionals will take care of sorting, washing, drying, folding, and putting away your laundry, as well as changing bed linens.

Light Housekeeping

Keeping up with household tasks can be challenging enough when you’re in good health, but when injury, surgery, or aging has limited your mobility, you may find that you need help in order to remain at home while staying on top of chores.

We offer light housekeeping services for elderly and infirm patients through our non- medical side of our agency. Our home health aides can help maintain a clean and safe environment by assisting with chores that may include vacuuming, dusting, doing dishes, taking out trash and recycling, cleaning bathrooms, and making beds.

Shopping / Errands

Getting older, illness, or surgery can make it difficult to run errands or get out to the store. If you or a loved one has trouble walking without assistance or shouldn't be driving, you may worry about running out of milk or eggs, or even toilet paper. Or maybe you can't get to the post office.

Through our non-medical services, we offer help with shopping and running errands for patients who are unable to do these things themselves. In addition, we can work to set up store accounts with local retailers where available, so you don’t have to worry about having enough cash on hand when our home health aides go out to shop for you. And, we can set up bill-paying services with your bank so missing a payment is not a concern.

Meal Preparation

If you or a loved one is recuperating from an illness or surgery, the demands of grocery shopping can be overwhelming. Through our nonmedical service, we can help you or a loved one remain at home, while still eating properly. Our home health aides will do the food shopping, prepare the meals, and handle kitchen clean-up. Our staff will also accommodate any special dietary restrictions or requirements. If this service is for an elderly loved one, you’ll also have peace of mind knowing that he or she won’t forget to turn off the stove.
